Accident Forms

• This form is used to record any minor accident a child has at JINS nurseries/ECCs.
• It must also be completed when a child arrives with evidence of an injury on their body. In this case, all precise information will be entered once the parent or caregiver has explained how the injury occurred. The parent/carer must sign the form before leaving the child.
